I'm 勝手に making a survey for our foreign national SHIMIN about how we can better support them in ENG and Yasashii JPN
If you all have any other suggestions to include in any of the question categories (particularly #4!!) I would be very grateful
All the categories also include a "other", write-in option

Survey on English/やさしい日本語(にほんご) Support at the City Hall
Thank you for visiting us at the City Hall. In order to learn how to better support our foreign residents in English/やさしい日本語(にほんご), ○○ City has created a questionnaire to ask for the direct input of our foreign residents. If you have the time, we would be deeply grateful if you could answer some questions in this survey.
1.When you come to the City Hall to complete an application (etc.) what things do you find difficult? (Select as many answers as applicable)
□ There are not enough guides/written materials in English/やさしい日本語(にほんご) about the applications I need to complete
□ I don’t know where to go/not enough visual information/maps in a language I understand
□ I understand (some) Japanese, but find it hard to understand employees (too fast/use too many difficult words/keigo (polite speech))
□ I understand (some) Japanese, but have a hard
time reading and understanding official city
applications (etc.)
□ I don’t understand (parts of) the Japanese system of the program/insurance/etc. I am applying for □ I don’t know where to find information inEnglish/やさしい日本語(にほんご)
